Have your say! Sunderland City Council wishes to invite you to take part in a discussion about leaving hospital.
Many patients discharged from hospital will not require ongoing care however, some patients will require further support, either on a short-term basis to support rehabilitation and recovery, or on a longer-term basis to meet ongoing care needs. These more complex discharge arrangements are likely to be lower in number but will require effective planning and co-ordination.
Sunderland City Council are reviewing how effectively health and social care services are working in partnership to support timely discharges from hospital and promote independence in community settings.
This event is an opportunity for you to hear about services for people being discharged from hospital and to contribute your views and experiences.
The event begins at 9.30 am and closes at 12.30 pm with a buffet lunch.
The event is free but you must book to attend. Places are limited and will be allocated on a first come first served basis.
